I believe haunted houses can be problematic just like things as simple as physical dangers.
Physical dangers include being trampled, slips, trips and falls, uneven and wet floors, crowd related injuries/overcrowding. Imagine having a reflex that causes you to hit someone, either the actor or someone attending a haunted house as well, Poor lighting causing injuries. A visitor may injure themselves or others trying to escape a frightening situation.
Panic-induced injuries can result when someone is so scared they cause panic attacks, which could come from both sides -- the actor and the visitor. The visitor might get scared by the actor, so it triggers an attack, or an actor might get scared because of how the visitor reacts.
Special effects mishaps: Flickering or lights move very frequently can trigger seizures with people who might be photosensitive. Special effects like lights might have an electrical hazards from manufacturing and might cause things like an outage or even a fire.
Health-related issues: Triggers like a “Fight or Flight” (adrenaline rush) stress response can increase blood pressure and heart rate. People who have health issues like heart conditions or problems can trigger asthma and panic attacks.
Haunted houses can trigger survival instincts and bad paranoia. For example, if you’re walking through the woods at night alone and hear a strange noise, naturally you're going to feel frightened and paranoid. When a person feels overwhelmed by something, their thoughts begin to race, which can cause them to see things. This can ultimately ruin their experience because they are paranoid.
Haunted houses are known for making visitors that attend related events act irrationally. Something as simple as going into a haunted house and seeing the eerie decorations can cause patrons to act impulsively because it messes up their mind.
After-effects: Lingering anxiety is a known anxiety to feel after attending a haunted house. After leaving a haunted house, this experience can replay in your head for days depending on how scared you were, and the memories from the haunted house can trigger anxiety.
